You can use the self-assessment enquiry online form to find out if we can provide you with help or support in your daily activities.
The self-assessment enquiry form is for people 18 years or over who are finding it difficult to manage every day activities. For example you or someone you know may be having difficulty managing steps or stairs, getting in and out of bed or some aspects of personal care.
You will be asked questions to help us understand your needs and what, if any, equipment or services we can provide to help you.
We can only provide services if needs are assessed as critical or substantial - please view our Fair Access to Care policy for more information.
If you have capital savings above £23,250 you will be considered to have sufficient resources to fund your own care needs. This means that you will need to pay the full cost.
