Transition Protocol and Pathway official launch - 22 August 2017

Do you, or members of your team, support young people with Special Educational Needs and/or Disabilities? 

From Tuesday 22 August 2017, all professionals will need to use the new Shropshire Multi-Agency Transition Protocol document together with the Transition Pathway document when supporting young people and their families through the transition process.

The Protocol and Pathway are to support all the professionals and agencies in Shropshire that have a responsibility in ensuring young people with Special Educational Needs and/or Disabilities (SEND) make a successful transition into adulthood. This includes Education, Health and Social Care professionals involved in planning and commissioning services as well as those delivering them. 

The Transition Protocol and Transition Pathway have both been created and developed by members of teams in Education, Health and Social Care who support young people with SEND. Ensuring that young people’s aspirations, thoughts and feelings are at the centre all processes that are carried out.

On Tuesday 22 August 2017  

Come along to one of the drop-in sessions to collect your copy of the Transition Protocol and Pathway.

You will also get the opportunity to meet Kelly Kubilius Transition Officer, Shropshire Council and members of the group who created and developed the Protocol and Pathway. All will be on hand to answer questions or queries you may have.
