What is a support plan?

If, following an assessment to determine your needs, you're eligible for our support, you'll have the opportunity to design a support plan.

Essentially, your support plan will detail the following:

• How your needs will be met
• How you propose to meet specific outcomes
• What it is you need to have in place to succeed

Once completed, your support plan will give us an indication of any proposed costs that may be involved to meet your needs. At this stage, we refer to this as an indicative budget.

Your support plan and indicative budget will need to be formally approved and signed by both you and us before you can formally make any arrangements. Once approved you'll be allocated your personal budget. You'll need to consider how to manage the budget to ensure that it operates in the way you need it to.
