Education, health and care plan
What is an education, health and care plan (EHCP)?
If your child’s special educational needs are complex and/or severe and, despite receiving additional help through the graduated approach hasn't made progress, you or your child’s school can request that we complete an education, health and care assessment in order to determine whether or not it is necessary to issue an EHCP.
What is an EHC needs assessment?
We undertake an education, health and care (EHC) needs assessment to determine whether an education, health and care plan (EHCP) is required to meet the special educational needs of a child or young person with SEN. An EHCP will be necessary where the assessment identifies that a child or young person requires access to specialist provision for a prolonged period of time in order to meet their special educational needs, and to support them towards achieving greater independence in preparation for adulthood.
How to request an EHC needs assessment
The procedure and forms for requesting an education, health and care needs assessment can be found in the 'requesting an assessment' section of these webpages.
If your child is in an early years setting you'll need to request an assessment as your child’s parent. Your child’s setting will, however, be able to support you in doing this.
