4. Ophthalmology Services
To receive a presentation on the reasons for the recent suspension of Ophthalmology Services and the steps taken to ensure the clinical safey of the resumed service.
The Joint Committee will seek assurances on the clincial safety of the service and the safeguards put in place to mitigate the risk of further events.

5. Shrewsbury and Telford Hospital NHS Trust: Foundation Trust Application
To receive a presentation from senior Trust Officers on the Foundation Trust applciation process, including the key milestones and the changes resulting from the Health & Social Care Act 2012.
The Joint Committee is recommended to agree whether a further formal period of consultation should be undertaken.

6. Head and Neck and Surgery Recloation
To receive a verbal update on the early tranfer of Head and Neck and other surgical services. This item will also include an update on the wider reconfiguration programme following the meeting of the Strategic Health Authority Board on 24 May 2012.
