You recently attended a meeting to discuss my concerns about your performance on the xxxx. At the meeting we talked about the issues and the impact these are having, as well as possible reasons for your performance not meeting the required level.
As a result, I have decided to issue you with a formal warning.
I'll arrange for us to meet again in xx months, and at this point I would like to see an improvement in the following areas:

I must inform you that if I don't see an improvement by the time we next meet I'll consider issuing a further formal warning.
You have the right to appeal against this decision. If you wish to appeal, please submit your notification of appeal within ten working days of receipt of this letter to (insert name, job title) Shropshire Council, (insert address). You should clearly state your grounds for appeal in your letter of appeal.
I enclose a second copy of this letter and ask that you sign and return it as acknowledgement of receipt.
